The CareFirst BlueCross BlueShield Community Health Plan Maryland (CareFirst CHPMD) Case Management team is here to support you and help coordinate your care. Whether you need resources to help manage a chronic condition or have any questions about your care, your case manager is here for you.
Your Case Manager will:
- Help you set goals and make a care plan to reach them
- Make sure you get the care you need when you need it
- Make sure you have what you need to get care, such as transportation to and from the doctor
- Find ways to help you manage your symptoms so you can feel better
- Answer questions about your health and Medicaid benefits
- Answer any questions you may have about your health over the phone—if you want us to visit you in your home or at your doctor’s office, we can do that too

CareFirst CHPMD programs are voluntary and are provided at no cost to you.

Baby Steps Program
The Baby Steps Program is a wellness program designed for pregnant people. If you have any questions or need help getting into care, please call Member Services at 410-779-9369 or 800-730-8530; (TTY: 711).
CareFirst CHPMD provides:
- Prenatal visits with $0 copay
- Free breast pump
- Prenatal vitamins
- Nutrition counseling
- Support quitting smoking
- Over-the-counter medicines and supplies
- Hospital stays
- Free meal service for members who are 20–28 weeks pregnant
It is important to schedule your first OB appointment as soon as possible. This will ensure a healthy pregnancy. Your first prenatal visit should be in your first trimester of pregnancy or within 42 days of enrolling with CareFirst CHPMD.
Services that may be done at your first visit:
- Early sonogram to confirm your pregnancy and due date
- Hear your baby’s heartbeat
- Blood work to determine pregnancy risk factors
If you haven’t already, please schedule your first OB visit today.
Regular OB visits will be scheduled:
- Every 4 weeks for the first 6 months
- Every 2 weeks for the 7th and 8th months
- Every week during the 9th month
The frequency of visits is determined by individual needs.
OB Case Management services
CareFirst CHPMD will work with you and your doctors to get you the services, resources and one-on-one coaching you need for a healthy pregnancy.
- Make sure you are getting the care you need when you need it
- Provide you with pregnancy health information
- Assist you and your doctor to arrange any special consultations or tests required to make sure you continue to have a safe and healthy pregnancy
- Connect you to community resources like Special Supplemental Nutrition Program for Women, Infants and Children (WIC), Substance Abuse programs, and Childbirth Preparation, Breastfeeding and Newborn Care classes
