La Clinica Del Pueblo: CareFirst Is A Health Promoter

Riverdale, MD

CareFirst funds contributed to the expansion of a new facility that is increasing access to care for the growing Latino and under- and uninsured populations in the Riverdale area of Prince George’s County, Maryland.

“When [Daniel] was informed by his provider that he was pre-diabetic, he panicked. His dad had passed away several years prior from diabetes, and his mother had suffered several amputations from diabetes complications. As a result of just eight sessions working with a health educator, health promoters and peers, Daniel lost 23 pounds. His story illustrates the success of our team approach, which would not be possible without the high level of interest from community members to become health promoters.”

Food & Friends: Providing Meals To Those In Need

Washington, D.C.

With support from CareFirst, Food & Friends has increased its ability to provide fresh produce to 3,400 critically ill clients on a regular basis through the organization’s Groceries-to-Go program.

"Living with cancer is challenging – no matter the circumstances. But for those with a limited income, or no income, a cancer diagnosis can present seemingly insurmountable obstacles. Connie, 65, is one of over 2,000 breast cancer clients to receive meals or groceries from Food & Friends. At her lowest point, Connie would go the entire workday without eating. ‘I was living on the edge,’ Connie said, ‘until Food & Friends came along.’ She now receives four bags of fresh food and pantry staples through [our] Groceries-to-Go program twice a month.”

Unity Health Care: Helping Get Patients From Hospital To Home

Washington, D.C.

CareFirst’s support for Unity’s Transition of Care Management Program provides intensive care coordination services for recently discharged emergency department patients, including individuals identified as hospital super-utilizers.
