Select a Primary Care Physician (PCP)
When you enroll in BlueChoice, you must select a PCP or your claims may be denied. Selecting a BlueChoice PCP is easy, just follow these steps:
- Find a Doctor online
- Inform CareFirst of your selection by:
- Calling Member Services at 410-654-8675 or 1-800-445-6036
or
- Downloading, completing and returning the PCP Selection Form(40KB, 2 pgs.)
