BlueChoice HMO members will be upgraded to CareFirst Exclusive Provider Organization (EPO) on July 1, 2009 if no other plan is selected during open enrollment.

Change Your Primary Care Physician (PCP) - Plan Ends June 30, 2009.
As a current BlueChoice participant you can change your PCP at any time. You must always have a PCP selected or your claims may be denied. Selecting a BlueChoice PCP is easy, just follow these steps:
- Find a Doctor online
- Inform CareFirst of your selection by:
- Calling Member Services at 410-654-8675 or 1-800-445-6036
or
- Downloading, completing and returning the PCP Selection Form (40KB, 2 pgs.)
