Problem with CMS-1500 (08-05) Paper Claim Forms
The Centers for Medicare & Medicaid Services (CMS) announced that there are versions of the CMS-1500 (08-05) form being sold by print vendors, specifically the Government Printing Office (GPO), that were formatted incorrectly. The GPO determined that the files they received from the National Uniform Claim Committee (NUCC) were improperly formatted, therefore printed incorrectly.
