Submit your NPI to CareFirst and CareFirst BlueChoice

It is critical for providers to begin sending their NPIs to health plans in advance of the NPI compliance date of May 23, 2007.

To submit your NPI to CareFirst and CareFirst BlueChoice, visit the Providers & Physicians section of www.carefirst.com and click on National Provider Identifier (NPI) in the Solution Center to find the NPI Submission Form (52KB, 2 pgs.)PDF

Download, complete and return the NPI Submission Form to submit your National Provider Identifier (NPI) and NPI-related information to CareFirst BlueCross BlueShield and/or CareFirst BlueChoice, Inc.
