Spiriva, Celexa Change Tier Levels
Beginning Aug. 1, 2004, the tier status of two drugs will change on the CareFirst BlueCross BlueShield (CareFirst) preferred drug list (formulary).
This drug is moving from a non-preferred or Tier 3 level to a preferred, or Tier 2 level, and will result in a lower copay for members with a 3-tier benefit who are prescribed this drug:
- SPIRIVA: Used for the treatment of bronchospasm associated with chronic obstructive pulmonary disease, including chronic bronchitis and emphysema.
This drug is moving from preferred, or Tier 2 level to non-preferred, or Tier 3 level, and will result in a higher copay for members with a 3-tier benefit who are prescribed this drug:
- CELEXA: Used for the treatment of depression.
Current members that are prescribed Celexa will continue to receive this drug at the current Tier 2 level until Nov. 1, 2004. At this time, there is no generic equivalent for Celexa. It is recommended that members change to Lexapro (escitalopram), a Tier 2 drug, which contains the most active form of the drug Celexa (citalopram) currently represented.
