Enroll Online in Free Case Management and Great Beginnings Programs
Eligible members can now enroll online for two additional free programs:
- The Case Management Program, developed to assist members with a variety of complex or long term health conditions
- The Great Beginnings Program, a case management program for expectant moms
Enroll in the Great Beginnings Program if you are pregnant or are thinking about getting pregnant. This free, voluntary and confidential program provides you with information and resources to help you and your baby throughout pregnancy.
Enroll in the Case Management Program, if you:
- have complex medical or surgical health issues
- have chronic or long-term health care needs
- require home care services
- are in rehabilitation, hospice or living in a skilled nursing facility
- have children with congenital or neurological disorders
Examples of conditions that are eligible for case management services include trauma, cancer, MS, stroke, high blood pressure and eating disorders.
Enrolling in either of these programs afford you a registered nurse case manager, who will work with you and your provider to coordinate all of your health care needs. The case manager can navigate the complex health care system to ensure that all of your health care needs are addressed. Your case manager will support you and your family by:
- communicating with your health care providers to develop a treatment plan specific to your care needs
- coordinating necessary services with network providers
- providing community resources, including classes and support groups
- working with your available benefits to meet all of your health care needs
