Qualify for Your Healthy Reward

HealthyBlue rewards you for staying healthy or getting healthy! You can earn up to $300 for yourself or up to $700 for your family.

Here's how it works:

  • Complete your online Health Assessment (if you are 18 or older).
  • Select your personal Primary Care Physician (PCP).
  • Visit your personal PCP for a Health & Wellness Evaluation.

That's it! If you qualify, your Healthy Reward notification will be mailed to you. If you didn't qualify, you can work with your personal PCP to improve your health and have a requalifying evaluation later during your plan year.

Note: You must have your initial personal PCP visit and evaluation within 90 days of your new plan year.

In addition to completing the Health & Wellness Evaluation Form with your personal PCP, you and your family members (who are 18 and older) are also required to complete the online Health Assessment. It's simple and easy. Just log in to My Account and click on the link called Health Assessment and answer a series of health and lifestyle questions.

A personal PCP who knows you is an essential part of the HealthyBlue plan. Your personal PCP will manage your primary care, and when needed, refer you to a specialist. Your personal PCP must participate in the BlueChoice provider network and specialize in family practice, general practice, pediatrics, internal medicine or OB/GYN.

Your personal PCP:

  • Knows and understands all your health care needs.
  • Works with you to complete your Health & Wellness Evaluation.
  • Develops a Healhy Action Plan to improve your health, if needed.
  • Helps determine when you need to see a specialist, select that specialist and provides you with a referral.
  • Coordinates care and treatment with CareFirst specialists and Blue Distinction Centers of Excellence.

Health & Wellness Evaluation – What You Need to Know!

When you visit your personal PCP, he or she will need to complete a Health & Wellness Evaluation. Lab work and certain screening results will be sent to your personal PCP who will then complete your Health & Wellness Evaluation Form. After you both sign it, your PCP will send the form to CareFirst.

The lab work and specified screenings are required to qualify for your Healthy Reward. If your personal PCP does not have a record of the lab work and screenings on file, please bring the dates of the following screenings or immunizations with you.

To qualify for your Healthy Reward

Cancer Screenings Childhood Immunizations Health Measures
Cancer screenings, within required timeframe, based on the following:*
  • Cervical Exam –
    Women 18+
  • Mammography –
    Women 50+
  • Colonoscopy –
    Men and Women 50+
 Childhood Immunizations such as:**
  • Mumps
  • Chicken Pox
  • Measels
  • Rubella
 Specific measures help your personal PCP evaluate your current health, as well as your potential for developing other conditions. During your Health & Wellness Evaluation, your personal PCP will review and discuss the following:***
  • Smoking Status
  • Blood Pressure Reading
  • Influenza Immunization
  • Cholesterol Levels
  • Body Mass Index (BMI)

*** If you've completed these tests within the last six months and your PCP can get the results, you may not need to take them again.
