1. Locate and select an Individual Select Dental HMO primary care dentist:

  • Select "Individual Coverage"
  • Select "Maryland or Virginia (for DC and Virginia plans)"
  • Select "Provider Directories"
  • Open the IND-20 provider directory
  • Choose a dental provider.
  • Call Dental Member Services at 888-833-8464 with your provider selection.
• By Telephone:
  • Call Dental Member Services at 888-833-8464 for assistance in locating a provider.

2. At the time of service, show your Individual Select Dental HMO member ID card.
3. Pay the copay for services received.
4. The dentist will submit all the necessary forms.
