CareEssentials, CareFirst’s care management program, is a unique program that focuses on the specific needs of our members, using innovative techniques to promote prevention, identify risk cases and manage patient care to avoid costly treatments.

CareEssentials care management components include:

We provide programs for members diagnosed with coronary artery disease, congestive heart failure, diabetes, cancer, and asthma. These programs are designed to help you better understand and manage your condition. Your CareEssentials Program benefits include:
