Preferred Pricing:

• $895* per eye for traditional LASIK includes:
  • Pre-operative exam
  • The LASIK or PRK procedure
  • Post-operative care
  • One year retreatment warranty
• $1,320* per eye for Custom LASIK
• $995 per eye for Conductive Keratoplasty (CK)
• IntraLase (bladeless LASIK) available when offered by participating provider- call (877) 285-2010 for locations
• 100% patient financing is available upon approved credit

* Fee structure includes surface ablation procedures (PRK, LASEK, Epi-LASIK) as necessary and as offered by participating providers.

How the program works:

1. Call QualSight toll-free at (877) 285-2010.
2. QualSight will answer your questions, pre-screen for the procedure, and perform a zip code search to pull a list of providers in your area.
3. You will pay QualSight a fully refundable deposit or select a flexible financing option, which will waive the deposit requirement.
4. QualSight will schedule the pre-operative exam over the phone.
5. After the appointment has been scheduled, QualSight will send you a confirmation e-mail complete with the provider's contact information and directions to the office.
6. Present your ID Card and confirmation e-mail at the provider's office to confirm pricing.
7. The provider will perform a comprehensive exam. If you are a candidate for the procedure, the provider will schedule a date to provide service. If you are not a candidate, the provider will refund your deposit if you paid one.

Retreatment Warranty:

• One year retreatment warranty is included within one year of the surgery date.
• Participating providers may offer eligible members the Lifetime Assurance Plan, which includes retreatments at no cost if medically advisable. Provider participation may vary.
